To fulfill our services to you, we are authorized to collect personally identifiable information (PII) from you by the exchanges of the Affordable Health Care Act and other agencies. Any PII we collect is used to enroll you in the coverages or services you select.

If you choose to give us PII, we may share this information with enrolling insurance entities, the Social Security Administration or exchanges of the Affordable Care Act. PII is used or disclosed only under the following circumstances: insurance enrollment, obtaining disability payments or our further representation of you or your dependents. 

All information is protected by Federal and state guidelines.
